Life Without Barriers is seeking a compassionate and organised Case Manager in Toowoomba to support young people transitioning to independence. This is a permanent full-time role that involves delivering person-centred case management and developing support plans for young individuals aged 15+.
As a Case Manager, you will address trauma impacts while collaborating with various stakeholders to enhance positive outcomes.
